    It was 1968, the same year Craig was born and said his Bond was born in Germany. As for the actual date in 1968 of his birth, I don't know.

    Yup, it was a slang word for the mushroom cloud created by an atomic bomb. The other answer is that it was his wife's nickname for him, although that's possibly an urban myth.

    I've just been reading 'The Letters Of Ann Fleming' and can confirm she actually referred to Ian as 'Thunderbird' rather than 'Thunderball' to her literary friends, on account of his passion for the American car model.
